Community Nursing provides clinical healthcare services in your home or local setting. This includes wound care, medication management, catheter care, health monitoring, chronic illness support, and more. It’s designed to deliver quality medical care outside a hospital environment, promoting comfort and independence.
Community Nursing is available to individuals of all ages with medical or health needs who prefer to receive care at home. This includes NDIS participants, people with chronic illnesses, disabilities, post-surgery recovery needs, or mobility challenges. Services are often funded through the NDIS, aged care packages, or private arrangements.
Our trained nurses assist with:
Wound and pressure injury care
Medication administration and monitoring
Diabetes and chronic disease management
Continence care (catheter/stoma)
Post-hospital recovery
Health assessments and vital signs monitoring
Care plan development and coordination
Yes, if you have nursing support needs related to a disability, the NDIS may fund Community Nursing under “Capacity Building – Improved Daily Living” or “Core Supports.” We can help determine your eligibility and provide NDIS-compliant reports to support your funding requests.
You can contact us directly, or be referred by your GP, Support Coordinator, hospital, or aged care provider. Once we understand your needs, we’ll schedule a nurse visit and tailor a care plan to suit your health goals and daily routine.
